Any personal information collected is inaccessible to anyone who is unauthorized to use it and our systems are designed to prevent the loss, misuse, disclosure, or alteration. Information collected will only be kept for as long as it is required.

Any information about an identifiable individual is considered “personal information”:

We do not sell your personal information. Your information will be released in the form of a medical examination report to the requesting/referring party.
